Working alongside the HOME Programme Coordinator and delivery team, this role will be at the forefront of receiving referrals that come into HOME. The Triage & 1-2-1 Project Worker will be responsible for following the day-to-day HOME referral process, including logging all new referrals on Lamplight and responding to them in a timely fashion. They will offer initial meetings to young people and parents to ascertain whether HOME is an appropriate service for them and if so, which programme they would be most suitable for. They will also ensure that young people, parents, carers and professionals receive a range of signposting information regarding the wider support available in Barnsley. Where necessary, this post will work with young people on a 1-2-1 basis if, where we consider they need some short-term support to be able to access HOME’s wellbeing group programme or external services. The right candidate will need to be extremely organised, be able to manage their time effectively and have excellent administrative and IT skills.
