Help to shape future services at SWYPFT - Barnsley

 

South West Yorkshire Partnership Foundation Trust (SWYPFT) are a specialist NHS Foundation Trust, who mainly provide community, mental health and learning disability services to the people of Barnsley, Calderdale, Kirklees and Wakefield and other parts of South and West Yorkshire.

 

 During the pandemic, all NHS services have had to adapt and find new ways of working.  As a Trust, they have done the same. OPTIONAL - If you want to hold a group conversation and send your feedback, you can find an engagement presentation here and well as a feedback form and equality monitoring form.  Just run the session and send back the data capture form included with your feedback under each question heading.  They would also be grateful if you could ask those attending to complete an equality monitoring form. This helps understand who they have included and where there are gaps of people they still need to hear from.
