As Link Worker Service Manager, you will be responsible for managing the PCN Link contract, developing and evaluating the project and liaising with key stakeholders and partners to ensure that the service is delivered in line with best practice and commissioners’ requirements. A key part of this role will be building relationships with key staff in the PCNs, attending relevant meetings and being part of the wider network team.
You will also be responsible for managing a small team of PCN Link Workers, based within specific PCN localities and GP practices, providing strong leadership to motivate, challenge and support Link Workers to meet their contractual, financial and performance targets.
As well as having experience of building relationships at a senior level within a range of organisations or service providers, you should also have an understanding of the wider determinants of health including social, economic and environmental factors. As well as an ability to communicate effectively, both verbally and in writing, with a wide variety of partners and stakeholders you should have experience of managing a team and the ability to inspire and motivate others. Experience of working directly in adult health and social care, public health or community development context would be advantageous but not essential.
The successful candidate must understand the need for robust data protection and confidentiality, mindful of safeguarding issues and be willing to work flexible hours when required to meet work demands. Being able to travel around the Borough is a key requirement of this role.
