The Rotherham Social Prescribing Service works with GP Practices and Mental Health teams to support adults with long-term conditions and mental health issues to tackle barriers that prevent them from self-managing their health and wellbeing and becoming connected to their local community.  A key aspect of the role is to assess the non-clinical needs of referred patients and, using a support planning approach, to provide a gateway into a wide range of support services and activities, primarily in the voluntary and community sector.  This is not a Support Worker role and as such does not involve providing ongoing or long-term face-to-face support for patients.  Voluntary and Community Services Advisors will assist people to access activities, services and support within their local community to develop healthy and supportive networks.
